WHAT TO KNOW:

• Children must be 5 years old by December 1, 2026.
• You'll register at your home school, just contact them for an appointment
• It’s IN PERSON only: we’re sorry, there’s no online option available.
• Print out and complete the registration packet (single-sided, please!) and bring it (with the required documents) to your appointment.

NYS Required Health Examinations

• Student health exams are required when new to the District and in grades K,1, 3, 5, 7, 9 and 11.
• You can use the NYS Required Health Examination Form, or you can just provide your physician’s form if it has the same information.
